Formsite

A veteran results focused builder that is steady, capable, and a little plain.

Formsite has been collecting form data since the late 1990s and it shows in both directions: it is stable and feature complete, but the interface and design options feel utilitarian. Plans are organized around the number of forms and the results allowed per form, plus add ons for sub users, file storage and merged PDF or Word documents. It is a solid choice for registrations, applications and surveys where reliable data capture matters more than visual flourish.
